The Nursing Home Completes the "Reading2Connect Nursing Home Profile"

The nursing home or senior care company decides to bring the Reading2Connect® LTC Program to their community.

Nursing home contacts Reading2Connect and asks for the status of their state's CMP Program.

If Reading2Connect replies that their state has an active CMP Program, then the process begins.

Nursing Home completes the "Reading2Connect Nursing Home Profile" and submits to Reading2Connect. See button below.

Reading2Connect Completes the CMP Grant Application

After reviewing the "Nursing Home Profile" and confirming eligibility, Reading2Connect® completes the CMP application at no cost.

Reading2Connect sends the completed application to the nursing home for review.

Reading2Connect also provides nursing home with state contact information to submit the application.

Nursing home approves the application.

The nursing home is the applicant and grantee, responsible for communication with CMP review agencies during the review process.

If approved, the nursing home receives funding to implement the Reading2Connect® LTC Program.
