Reading2Connect will complete your CMP Grant Application at no charge.
Most states have Civil Monetary Penalty (CMP) Reinvestment Programs which hold funds designated for Certified Nursing Homes of that state. To access those funds, nursing homes must submit an application for funding to bring a particular resource to their nursing home. Each state has different application requirements and processes. The application process is usually quite time-consuming.
Reading2Connect offers a free service to nursing homes. Reading2Connect will complete your CMP application so that you may apply for funds to bring the Reading2Connect LTC Program to your nursing home. See steps below.

The Nursing Home Completes the "Nursing Home Profile"
First step is for the nursing home or senior care company to decide if they would like to bring the Reading2Connect LTC Program to their nursing home(s).
View and download the Reading2Connect LTC Program details below.
Next the nursing home (or senior care company) completes the online Reading2Connect Nursing Home Profile Form and submits it to Reading2Connect. See form below.

Reading2Connect Completes the Grant Application
After reviewing the Nursing Home Profile, and affirming that criteria have been met, Reading2Connect then completes the CMP application as a free service.
Our grants team knows the CMP requirements and processes for each state. We will review your state's application process with you.
Reading2Connect then sends the completed CMP Application to the nursing home for review. When the nursing home considers the application to be satisfactory, then the nursing home submits the application to the state agency that manages CMP Grants.
